I declare that I'm not aware of any health conditions that would restrict my child's ability to partake in any of Cultivate Beyond Sports' activities. I acknowledge all the risks involved and will bear all consequences and expenses due to any injury that may occur. In case of an emergency, I grant any treatment to my child.

I also understand that my child may be photographed or filmed for the purpose of publication. I agree that such material or any reproduction of the same or any form are the property of the organization, and I relinquish any present or future reimbursement.

I release, waive and discharge the organization and all its members from any demands, loss, responsibility and all liabilities. I confirm that I have read, fully understand and agree to all the terms. I, the Parent/Guardian of the child give full consent to their participation in the program.
